Directions For Using Your Kit

Thank you for participating in the Sister Study Anniversary Biospecimen Collection! In the information packet included with your collection kit packet you will find—

  • Instructions for what to do first with your Anniversary Biospecimen Collection Kit.
  • General information on the questionnaires.
  • For each sample type (stool, urine, and blood): the specific kit contents, a brief description of the related questionnaire, and instructions for how to collect your samples. Please ignore instructions for samples that you are not providing.
  • Information about returning your samples to us.

What to Do First

  • Confirm that the name and address on the kit box that you received is yours.
  • Remove all gel packs and place them in your freezer so they will be frozen when you need it.
  • Check the contents of the Kit to make sure you have everything needed to collect your samples.
  • Put your Kit in a safe place where it will not be disturbed, and the contents will not get wet. It is best to keep it at room temperature.
  • If you are providing blood, ExamOne will contact you over the next several days to schedule your blood collection.

Sample Collection Questionnaires

You will fill out brief questionnaires for each sample type you are collecting. The data we collect using these questionnaires will provide important information that may explain some of your samples' biological characteristics and help us interpret the results of some tests. Please complete the questionnaires when the corresponding samples are collected.


Blood Sample Collection:

Blood Collection Kit Contents (for the phlebotomist, do not open)

  • 5 Blood collection tubes
  • 2 Absorbent sleeves for blood tubes
  • 1 Biohazard Bag
  • 1 Absorbent sheet
  • Phlebotomist checklist
  • Blood Collection Questionnaire (if not being completed online)

Blood Collection Questionnaire – This questionnaire asks about your use of medications, other agents, and various chemicals you might be exposed to that may influence what is found measured in your blood.

  • Please complete this on the day of your ExamOne blood draw appointment before the phlebotomist arrives.
  • This questionnaire can be completed online or on paper. If you elected to complete it online, you should have received an email with instructions on how to do this. If you elected to do this on paper, the blood collection questionnaire is included in your Anniversary Biospecimen Collection Kit.

For Blood Collection

  • A Licensed phlebotomist from ExamOne will draw your blood using the materials in your Kit. Please keep these materials in a place where you can easily find them and give them to the phlebotomist when they arrive.
  • Complete the blood collection questionnaire on the same day your blood is drawn.

Urine Sample Collection:

Urine Collection Kit Components

  • 6 Urine containers (100ml)
  • 6 Specimen bags
  • 6 Absorbent sheets
  • 6 Wet wipes
  • Urine Collection Questionnaire

Urine Collection Questionnaire – This questionnaire has questions that you will answer after you collect each of the 6 urine samples; the questions ask about the collection times and some other factors that may affect what is found in your urine.

  • Please complete the section of the questionnaire that corresponds to the day (1, 2, or 3) and time (morning or evening) immediately after you collect each sample.
  • This questionnaire should be completed on paper. The urine collection questionnaire is included in your Anniversary Biospecimen Collection Kit.

How to Collect Your Urine Sample

Please collect these samples starting at least 4 days before your blood draw appointment.

You will complete 6 urine collections over the course of 3 consecutive days. You should collect urine once each morning and once each evening for 3 days in a row. The morning and evening collections do not have to be at any specific time but should be spaced about 12 hours apart. You will use a new urine collection cup for each urine collection. The individual urine containers do not need to be labeled to show when each sample was collected.

Instructions for the Morning Collection:

  1. Before you collect your morning urine sample, wash and dry your hands thoroughly.
  2. Take the lid off the prelabeled urine collection container and avoid touching the inside of the container or lid.
  3. Hold the container under your urine stream and collect enough urine to fill the container about half full. Do not fill the cup completely.
  4. Place the lid on the urine collection container and close it tightly.
  5. If necessary, wipe the outside of the collection container with a wet wipe.
  6. Place the container inside the biospecimen storage bag, seal the bag tightly, and store it upright in the freezer.
  7. Remember to complete the section of the urine collection questionnaire that corresponds to the sample you collected.

Instructions for the Evening Collection:

  1. Use the toilet as needed throughout the day, but do not collect any urine until the evening.
  2. Before you collect your evening urine sample, wash and dry your hands thoroughly.
  3. Take the lid off the prelabeled urine collection container and avoid touching the inside of the container or lid.
  4. Hold the container under your urine stream and collect enough urine to fill the container to the marked line. Do not fill the cup completely.
  5. Place the lid on the urine collection container and close it tightly.
  6. If necessary, wipe the outside of the collection container with a wet wipe.
  7. Place the container inside the biospecimen storage bag, seal the bag tightly, and store it upright in the freezer.
  8. Remember to complete the section of the urine collection questionnaire that corresponds to the sample you collected.

Stool Sample Collection:

Stool Collection Kit Contents

  • 1 Absorbent sheet
  • 1 Pair Medium gloves
  • 1 ColOff® Stool catcher with instructions
  • 1 Tri-paneled perforated stool smear FOBT (Fecal Occult Blood Test) card with barcode label
  • 2 Smear sticks
  • 1 Biohazard bag
  • 2 Desiccant packs
  • Stool Collection Questionnaire (if not being completed online)
  • 1 Pre-labeled Envelope (To be used only if separate return mailing for stool sample is needed)

Stool Collection Questionnaire – This questionnaire asks about characteristics of your stool samples and your use of medications and other agents that could affect what is measured in your stool.

  • Please complete this on the day you collect your stool sample, which should be the day before or the day of the ExamOne blood draw appointment but before the phlebotomist arrives.
  • This questionnaire can be completed online or on paper. If you elected to complete it online, you should have received an email with instructions on how to do this. If you choose to do this on paper, the stool collection questionnaire is included in your Anniversary Biospecimen Collection Kit.

How to Collect Your Stool Sample

Please collect your stool samples either the day before or on the day of your blood draw appointment before the phlebotomist arrives.

  1. Bring your stool Kit materials to your bathroom. Keep all necessary materials within reach for easy access for when you are ready to have a bowel movement.
  2. Take out the FOBT cards and place them on a clean dry surface. DO NOT set the FOBT cards or applicator sticks on the edge of the sink, bathtub, or toilet. They should not get wet.
  3. Carefully open the ColOff® stool collection device so it is not ripped. Gently tear out the middle section of the fecal collection sleeve. Place the fecal collection sleeve around the clean toilet seat with the hole facing up, and gently push the bottom downwards. See the ‘HOW TO USE ColOff®’ instructions enclosed with the stool catcher if more detail is needed.
  4. Have a bowel movement in the fecal collection sleeve. Please do not urinate or to put toilet paper into the sleeve.
  5. Wash your hands thoroughly and dry completely. After your hands are dry, put on the provided gloves. If at any time during this process you accidentally get stool on your gloves, simply STOP and wash your gloved hands with soap and water and thoroughly dry your gloved hands, then continue the collection.
  6. Open the FOBT card. There will be six windows for stool application.
  7. Take a pea-sized sample of your stool with one end of an applicator stick. Apply a thin smear of stool inside each of the six windows on the FOBT card, being sure to fill the collection windows. You may need to scrape up more feces onto the stick to fill all the windows. For this study, it is not necessary to take samples from different areas of stool for each window as instructed on the FOBT card.
  8. Use the second applicator stick if the first breaks or becomes unusable.
  9. Throw the stick(s) in the trash.
  10. Close the cover of the card. Immediately place it in the biohazard bag that came with the Stool Kit materials.
  11. Add the two desiccant packs to the bag and seal.
  12. Remove gloves by pulling from the wrist opening to turn them inside out; discard gloves in trash. Wash hands thoroughly.
  13. Store the biohazard bag containing the FOBT card at room temperature, away from light, children, and pets.
  14. Remember to complete the stool collection questionnaire on the same day as you collect your sample.

Instructions for Returning Samples

Sample Return Kit Contents

  • Gel packs

General Instructions

  • The ExamOne phlebotomist will pack and ship blood samples and stool and urine, if collected.
  • Keep gel packs frozen.

Keep samples at recommended temperatures (urine frozen, stool at room temperature) until all have been collected and you are ready to ship them back.


Stool Samples Alone – if you are collecting stool only OR if you collected your stool sample after the blood was shipped

  • Place the filled FOBT card in the biohazard bag in the prelabeled cardboard envelope.
  • Seal the envelope and take to the nearest FedEx location.

Stool Samples with Urine

  • Follow the instructions for the return for urine samples below and place the filled FOBT card in the biohazard bag on top of the urine sample.

Urine Samples

  • Use the kit cooler to return urine samples.
  • Place the frozen gel pack on the bottom of the cooler.
  • Place the filled urine containers in biohazard bags on top of the gel pack.
  • Close the cooler with the Styrofoam lid.
  • Place any paperwork in the resealable plastic bag and put on top of the cooler.
  • Seal the box with the tape provided.
  • Attach the return FedEx label on the box making sure that the original label is either removed or covered.
  • Take the package to the nearest FedEx location.
